Looking at the model of care coordination, the primary goal is high-quality referrals and high-quality development to meet the six institute of Medicine "Quality Cavity," aiming for high-quality healthcare and assurance that all involved providers, institutions and patients have the information and resources needed to optimize patient's care. The model looks at care coordination from the prospect of a patient-centered medical home (PCMH). It includes major external providers and organizations with which a PCMH must interact like medical specialists, community service agencies, and hospitals and emergency facilities; and encapsulates the elements appearing to contribute to successful referrals and transitions. These elements include:

  • Assuming accountability
  • Providing patient support
  • Relationships and agreements are building among community agencies which lead to shared expectations for communication and care.
  • Developing connectivity through electronic information pathways to encourage timely and adequate flow of information between providers including community agencies.

It becomes challenging to counsel the patient when the doctor has prescribed lots of different medications and tests as when rarely meet or talk to each other. Failing to coordinate care may result in medication errors, unnecessary or repetitive diagnostic tests, and preventable emergency room visits. These errors and unnecessary tests contribute to unnecessary health spending.

According to research, it is estimated that inadequate care coordination has resulted in $25- $40 billion expenditure in 2011 due to unnecessary complications and hospital readmissions.

Importance of care coordination-

The Institute of Medicine identifies care coordination as an essential potential strategy for improving the validity, immunity, and adaptability of the American health care system. Well designed, targeted care coordination delivered to the right people can improve outcomes for patients, providers, and payers.

To better ordinate with the patient's care, it is essential to redesign the health care system for the following reasons:

  • Current health systems are incoherent, and processes vary between primary care sites and specialty sites. 
  • Patients are often confused about why they are being referred from primary to a specialist, how to make appointments, the next step after seeing a specialist.
  • A gap in communication is created between a specialist and primary care Physicians. Specialists are not clarified about the referrals reason or adequate information on tests which have already been performed. Primary care Physician often doesn't receive information about referral visit session. Inadequate information also leads referral staff with different processes and lost data, consequently directing in less efficient care.

The road ahead to heal the flaws of care coordination-

Based on previous AHRQ work, Care Coordination Quality Measure for Primary Care (CCQM-PC) is developing a conceptual framework for care coordination. The CCQM-PC contemplates filling the gap in the care coordination filed by more accessible and more comfortable methods in primary care settings. It is developed, subjectively tested, administered with patients from a diverse set of 13 primary care practices to comprehensively access patient’s perceptions.
